Use these forms when you are not able to connect electronically to Mayo Medical Laboratories. Timely and accurate processing of your order, specimens, and results are ensured by using these forms, which are specifically designed as backups to the online tools.
Important! Print the appropriate forms in advance so you have them on-hand in the event your network or PC is unavailable. You must include a downtime form with each specimen.
